A surgical guide can be used for immediate implant placement, especially when the case requires high accuracy. Digital planning with CBCT scans helps determine the ideal implant position before extraction. That said, not every immediate implant case requires a guide. Skilled clinicians may place implants freehand, particularly when socket anatomy and visibility allow precise control.
However, this course will teach you the complete digital workflow, from planning to guided surgery.
In conventional implant dentistry, immediate implant placement is not advised when primary stability cannot be achieved. This includes cases with severe bone loss, an absent or very thin facial bone wall, or poor bone quality or infection. This refers to a wide range of clinical cases and leads to patients being exposed to long treatments with multiple surgeries.
However, with the proven IDR technique of flapless bone reconstruction, treatment time can be reduced to 3 months in total and to one single surgical intervention. Sounds like magic, but it is not. The power of this IDR concept lies in active bone cells harvested from the maxillary tuberosity, which is the gold standard of autogenous tissue harvesting and at the same time very well tolerated by patients.
The fundamentals and latest scientific evidence of bone regeneration and healing is an added value of this course and will be taught by the world renowned bone researcher Luis Violin as a co-teacher of this course.
The IDR technique is not limited to the esthetic zone. While it is widely used in anterior cases to preserve soft tissue and bone contours, it can also be applied in posterior regions for the same reason, independent of single or multirooted teeth.
The main goal of the IDR techniques, besides prevention of tissue collapse, is to use the tremendous healing potential of the fresh extraction sockets and to support predictable healing, regardless of tooth position.
Postoperative care after IDR follows standard implant healing principles with close monitoring. Patients are usually advised to keep the area clean, use prescribed rinses, and take medications as directed. Chewing pressure on the implant site must be avoided during the first 3 months.
Follow-up visits are only during the close post-surgical phase necessary, then patients are instructed to return after 3 months or when they would feel an unusual sensation.
Implants used for immediate placement should support strong primary stability. Tapered, screw-type implants with deeper threads are often preferred because they engage bone more effectively in fresh sockets. A moderately to highly roughened surface is also ideal, as it promotes faster bone contact and early integration. These design features help compensate for the lack of healed bone at the time of placement.
